CCME Foundation Now Accepting Grant Applications from Nonprofits Improving Healthcare

North Carolina and South Carolina nonprofits can apply now for funding for projects that improve health care outcomes for individuals and communities.

Raleigh, NC – The CCME Foundation, a nonprofit organization supporting health care access and improvement in the Carolinas, is
accepting grant applications from eligible nonprofit organizations that work to improve health care for individuals and communities through measurable outcome improvement projects.The yearly grants range from $5,000 to $50,000 and fund projects that impact health care improvements and access for individuals and communities, particularly the uninsured and underserved. The CCME Foundation invites eligible
nonprofits to apply for funding April 1 through June 15, 2024. The Foundation will announce grant recipients in early August 2024.

“It is truly a rewarding feeling to give back to the people and communities who dedicate their time and energy to improve health care systems and peoples’ lives,” said Steven Martin, president and CEO of Constellation Quality Health, the nonprofit organization that supports the CCME Foundation. “We look forward to strengthening and broadening previous partnerships and building new ones with organizations across the Carolinas through our Foundation to continue this important work in 2024 and beyond.”

Constellation has collectively worked with health care providers, stakeholders, and advocates for more than 40 years. The foundation considers applicants with current 501(c)(3) status, including faith-based, hospital/provider/clinic, educational institutions, and community service organizations. The foundation funds programs with a focus on:

  • Healthy lifestyle and quality of life programs
  • Access to culturally and linguistically appropriate care
  • Consumer-centered care and decision-making
  • Health-related educational information
  • Patient safety programs

The CCME Foundation will not fund reimbursable direct patient care services, facility construction or remodeling of facilities, lobbying, and fundraising activities. Awardees are required to submit two online progress reports throughout the contract period. The CCME Foundation Board of Directors Review Committee oversees the grant process and determines recipients.
